Indications
Orlistat is indicated in the treatment of obesity and/or in the prevention of weight regain. Orlistat is also indicated in the treatment of dyslipidemia. Orlistat is also indicated in the treatment of the type 2 diabetes mellitus. Orlistat is also indicated for the treatment of patients with type 2 diabetes mellitus.
Dosage
The recommended dose of Orlistat is one capsule or sub-chewable tablets, taken once daily at the same time each day with or without food. The total daily dose for the treatment of obesity and/or dyslipidemia is one capsule or sub-chewable tablets, taken at the same time each day with or without food. The recommended dosing frequency is once daily. No dosage adjustments can be made at the same time.
Contra indications
The concomitant use of Orlistat and other antidiabetic medicines with Orlistat should not be considered contra-indications, and the combination of Orlistat and other antidiabetic medicines should be used with caution. Orlistat is contraindicated in patients with severe hypercholesterolemia, within 4 weeks of orlistat administration, and in patients with hyperthyroidism.
Side Effects
The most common (and possibly most serious) adverse reactions (incidence of 2-3%) associated with the use of Orlistat with other antidiabetic medicines are diarrhea (nine patients), abdominal pain (eight patients), weight loss (eight patients), flatulence (six patients), cholestasis (four patients), constipation (three patients), bloating (three patients), gas intolerance (one patient), oily stools (one patient), flatusitis (one patient), oily stools (one patient), oily stools (three patients), rectal bleeding (two patients), flatusitis (one patient), peptic ulcer (one patient), and hypersensitivity (one patient).
Mode of Action
Orlistat increases the level of lipase, an essential protein in the digestion of fat in the intestine. This leads to the production of triglycerides (a compound that is necessary for the breakdown of food). On the other hand, Orlistat reduces the level of free fatty acids (a substance that are produced by the liver) in the intestine, thus decreasing the amount of triglycerides and increasing the effectiveness of diet changes. The inhibition of the enzyme CYP2C19 increases the level of free fatty acids in the intestine and leads to the production of free fatty acids in the gut, which are then used for the purpose of the treatment of obesity and/or dyslipidemia.
Pharmacology
Orlistat is a substrate for CYP2D6. Therefore, Orlistat should be used with caution in patients with hepatic, gastrointestinal, or renal impairment.
Interaction
The co-administration of Orlistat and other antidiabetic medicines with Orlistat should be avoided.
Warning
It is not recommended to take Orlistat during pregnancy. It is recommended that there be restriction of the dose of Orlistat during pregnancy, and that regular monitoring of pregnancy and breast feeding is carried out.
